Amani-Trinity United Community Health Corporation
Amani~TUCHC was incorporated in February 2000 for the purpose of promoting the overall health, wellness and peace of African American individuals and families, and others in south side communities served. The organization has five program areas: 1)Synergy Family Resource and Support Center, offering individual and family professional counseling services; 2)HIV/AIDS education and prevention services; 3)education around end-of-life care and care for persons in hospice; 4)volunteer and respite services for persons chronically, seriously and terminally ill; and, 5)educational forums around memory loss, caregiving for the caregivers, advance care planning, and cancer awareness. While it is affiliated with Trinity United Church of Christ, Amani~TUCHC is a separate corporate entity with a board of fifteen which meets quarterly to oversee corporate programs and governance.
