Austin Coming Together (ACT) works with our 50+ member network and dozens of on-the-ground partners to carry out hyperlocal community-based engagement, building awareness and access to the vaccine for Austin residents. Since ACT began in 2010, we have grown to become the backbone support for Austin’s social service infrastructure, always focused on understanding community needs. We recently developed a response and recovery strategy that draws from our experience in identifying core issues and facilitating solutions through collaboration. We have operationalized a plan for advocacy, communication, resource development, and resource deployment. With this project, we will be able to expand our impact to more Austin residents.

Since 1972, El Hogar del Niño has proudly invested in developing our Pilsen community and the next generation of resilient leaders through our award-winning early learning programs and responsive family support services that promote stability & wellbeing so families can meet their full potential. Research supports what we know from nearly five decades of service: vulnerable families receive the strongest advantages from quality programs like ours - benefits that can improve life outcomes for generations. In partnership with the Trust, we will scale our impact to serve even more students experiencing trauma, open a Family Welcoming Center, and serve more families in need from a second location in McKinley Park.

Equal Hope (EH) is proposing a multi-pronged approach to address the critical needs of Chicago communities heavily impacted by COVID-19. Equal Hope Community Health Workers (CHWs), as trusted messengers in collaboration with other trusted messengers such as faith based leaders, will address vaccine hesitancy and misinformation about the COVID vaccine to increase vaccine confidence and uptake in Chicago's most vulnerable communities. The project's goal is to improve knowledge and attitudes around the COVID-19 vaccine so as to increase access and uptake of COVID-19 vaccine and eliminate COVID-19 disparities in areas of high COVID 19 incidence/mortality.

The Chicagoland Workforce Funder Alliance seeks additional resources for the Progressive Pathways for Post-Secondary Success Fund (Pro Path Fund). The mission of the Fund is to expand the universe of transparent and accessible Progressive Pathways to Post-Secondary Success. Progressive Pathways allow individuals to alternate between and combine periods of education/training and employment, and thereby progressively build toward college and career success over an extended period of time. Building on the selected accomplishments of the past year, and the financial support of 20+ workforce development and education funders, the Pro Path Fund looks forward to advancing the work of the Fund and progressive pathways ecosystem.

As the leading, homeless services organization on the west side of Chicago, Franciscan outreach provides, safe shelter, three warm meals, and access to essential resources to our most vulnerable neighbors. Continued support from The Chicago Community Trust will help us to reach Chicago's most marginalized people and empower them to gain the stability needed to move towards self-sufficiency. In addition to shelter services, all guests are linked to a case manager who through a trauma-informed lens link guests to vital housing resources, immediate needs, and specialized services such as access to critical resources for those who need specialized services such as health care, mental health counseling, and substance use disorder treatment.

Howard Brown Health requests $60,000 from the Chicago Community Trust (CCT) to support the agency’s vaccine services expansion in South and West Side neighborhoods. With support from CCT, Howard Brown will continue offering community-led vaccine education and distribution programming in areas disproportionately affected by the COVID-19. Throughout the project period, Howard Brown will 1) launch a city-wide vaccine social media and radio ad campaign tailored towards families, 2) provide education at community access points, and 3) and produce vaccine educational materials targeted towards LGBTQ youth. Howard Brown would be proud to partner with CCT to strengthen equitable COVID-19 response efforts.

This 2nd year is a refinement & continuation of the partnership with community organizers & emerging ALAANA journalists to produce solutions-based stories highlighting issues, challenges & ground level efforts for change.The incubator continues to serve as a training & preparation mechanism for emerging journalists of color, while building connections & trust with community organizations serving POCs in Chicagoland. Virtual townhall events are co-hosted with the community partners, using the completed episodes as tools for organizing & information exchanges. Adjustments include shortening the workshop from 12 to 8 weeks, reducing the number of participants from 12 to 8 (4 journalists, 4 activists), and fewer contact hours for the activists.
