    Awarded: Awarded Amount: $140,000

    Last year, Chicago House received transformational funding from the Sylvia Fund of the Chicago Community Trust to partner with two researcher/trainers to examine the peer health navigation model for service delivery. The partnership has allowed us to establish a new organizational approach embedding peer navigators into our comprehensive care strategy. This will help serve our clients better in the long term by increasing trust and ensuring retention in comprehensive care. In collaboration with the two leading experts, Chicago House will continue to study the impact and expand upon the lessons learned through the pilot phase of our implementation to improve housing and health outcomes for populations living with or vulnerable to HIV.
