Our organization Neighborhoods Uniting Kids in Excellence will engage the same youth group we've been working with over the summer three days a week to fulfill the grant requirements.We've expanded our space and have added more staff. We will continue to work our program Bridging the Gap between the Community and Police through N.U.K.I.E and operate three days a week for the Labor Day plus August gap. We looking to get funding to support our program for the Labor Day plus August gap. Will had continually incorporate free hair cuts for the youth and along with face painting.

BPA365 partners with New Hope Christian M.B.C. to provide anti-bullying training and meals for 40 youth (ages 14-18) community workers during the 1-week gap. BPA365 is partnering with Global Girls for ten youths (ages 12-17) to receive anti-bullying and video training to produce an intergenerational movie. BPA365 supports a "Back to School" event by donating school supplies and providing anti-bullying materials on Saturday, August 26th, from 10 AM to 3 PM for 100 youth (ages 5-18)

THE LITTLE VILLAGE COMMUNITY COUNCIL WITH ITS CULTURAL COMMITTEE WILL BE HOSTING A " LV MINI OLYMPICS" FOR CHILDREN AGES 5 TO 11 YEARS OLD. WE GOING TO HAVE FOOD/SNACKS & BEVERAGES AND ALSO PRIZES FOR CHILDREN AND YOUTH. A PARAMEDIC CAN GIVE A LESSON TO CHILDREN AND YOUTH ABOUT CPR AND THERE ALSO GOING TO BE AN ART TABLE SO THE CHILDREN AND YOUTH CAN COLLABORATE ON A BANDER. WE WILL BE HOSTING THIS EVENT AT LA VILLITA PARK IN COLLABORATION WITH PARK ADVISORY COUNCIL (PAC) ON SATURDAY SEPTEMBER 02, 2023. THE EVENTS WILL START FROM 1 PM-4 PM.

We have been running an after school and summer camp program for 7 years, We have staff, planned activates (Arts and Crafts, Music Lessons, Tutoring and Social Skills) and a Safe space (Community Center) for youths. We look forward to adding more days to accommodate the gap between summer camp and start of the school year. accommodate the youth in the community.

Joys Helping Hands will build community fun awareness by partnering with hundred's of youth and families during gap time and Labor day weekend. We plan to engage youths 3 days a week from August 7 through Labor day. These are some of the events we will be doing, Yoga classes at the FireHouse Community Arts Center 2111 S Hamlin, release and share breakfast 2149 S Drake Ave, fish fry game day, basketball hoop day, toss up taco Tuesday & ice cream social 3828 W Cermak Rd.

Each year we continue to build upon what we have done in the past and introduce new ideas. Earl’s Garden Mae’s Kitchen would like to use this additional opportunity to extend our programming during the gap time when the Chicago Park District has closed its programs and just before Chicago Public Schools are ready to open its doors for the new school year. • Week of August 7th & Week of August 17th • Youth will work three days a week Mon. – Wed. • Three hours per day • 9 a.m. – 12 p.m.

Sista Afya Community Care (SACC) is honored to submit this proposal to the Chicago Community Trust's Unity Fund. As an organization deeply committed to supporting the mental health of Black women in Chicago, we are excited by the opportunity to partner with a funder that shares our dedication to improving the mental health and wellbeing of Black women in Chicago. SACC is committed to leveraging community-driven mental health services, advocacy, and a dedication to service accessibility to bridge the mental health gap in the communities we serve. We believe that our beneficiaries should play a significant role in shaping our programs, we are devoted to creating a receptive environment for active community engagement. This engagement fosters a sense of ownership and connection. We take pride in our representation and inclusivity as a wholly Black-led and Black-serving organization. We ensure the voices and experiences of the Black community are at the forefront of our initiatives, resulting in strategies that are responsive, relevant, and catered to the unique needs of our community. In the pursuit of mental health equity, SACC operates three core programs: Thrive in Therapy, Community Care, and Developing Mental Wellness Warriors. These programs, ranging from free, tailored therapy services to topic-based workshops, group therapy, wellness classes, and community member training, are designed to provide comprehensive mental health support. Our track record of successful service delivery and the impact we have had on our community is demonstrated by both the quantitative outcomes of our programs and the heartfelt testimonials from the people we serve. SACC has recently seen significant growth, enhancing our capacity to serve our community and extend our reach to more Black women encountering barriers to mental health care. In the spirit of transparency and commitment to continuous improvement, SACC is devoted to systematically measuring and reporting our outcomes, including those specified by the Unity Fund. We recognize that data-driven evaluation is integral to enhancing our programs and increasing our impact. As such, we have developed a robust system for tracking our progress and evaluating our effectiveness through various outcome indicators. This allows us to monitor our performance and to make informed adjustments to our strategies and initiatives, ensuring the maximum possible impact on the communities we serve. We believe our work aligns perfectly with the vision and priorities of the Unity Fund. We are excited by the prospect of partnering with you to bring about a positive change in the mental health landscape for Black women in Chicago. With the support of the Unity Fund, we are confident that we can further our mission, extend our reach, and have an even more profound impact on the mental health of our community.
